I want to be able to create a form in Apptoto (with text, checkboxes, dropdown, free form text entry, signature boxes, etc.) that clients can fill out before or after their appointments. The reminder messages should easily include a link to the form if they haven't filled it out already. The data they enter should be written back to my CRM somehow.

It should be easy to see all of the people who have filled out the form.

The forms should be nicely formatted and brandable.
